Nomination Form

HALL OF FAME NOMINATION FORMS

Purpose:  To formally recognize those individuals who have graduated from Bath High School or have been contributing members to our Bath community that have excelled in athletics, community service or their professional careers.  

 

Eligibility:

 

*Any Bath High School alumni or contributing community member.  In order for alumni to be nominated, they must have graduated at least twenty years prior to their nomination.

*Nominees must have received local, state or national recognition for accomplishments in the category for which they are nominated. 

*The candidate must be of good character and in good standing with the community.

 

Required Documents:  A nomination form and two letters from individuals supporting your nomination must be submitted by March 31st to be considered for this year’s Hall of Fame ceremony.  Applications received after March 31st will be considered for the next induction ceremony.  Any nominations not selected will automatically be considered with the next cycle of nominations.  You can submit your nominations using the Online Nomination Form or print the printable Bath Hall of Fame Nomination Form and mail/email to Bath Administration Office  2650 Bible Road  Lima, OH 45801 or emailed to Missy Vandemark, Administrative Secretary, [email protected].
